ACTION NEEDED: Autism Needs Assessment (Appalachian District Health Department)

You are invited to participate in a survey about the needs related to the autism spectrum in the following North Carolina counties: Alleghany, Ashe, Avery, Mitchell, Yancey, Watauga, and Wilkes. Through this survey, we are seeking to understand the perspective of service providers, educators, and community members.

· I am a service provider/educator who works with individuals with, or suspected of having, an autism spectrum disorder (Asperger’s syndrome, pervasive developmental delay – not otherwise specified, autism, Rett’s syndrome, or childhood disintegrative disorder).

· I am a community member who may interact with individuals with, or suspected of having, an autism spectrum disorder (Asperger’s syndrome, pervasive developmental delay – not otherwise specified, autism, Rett’s syndrome, or childhood disintegrative disorder) in the course of my work or other community roles.
